It is difficult to definitively answer this question. Pitch-by-pitch speed records have not really been kept by Major League Baseball, and, of course, there were generations of pitchers in the early days of baseball who never had their pitch speed recorded. All of that being said, the fastest pitch on record is one thrown by left handed pitcher Aroldis Chapman of the Cincinnati Reds. In a relief appearance against the San Diego Padres on September 25, 2010 Chapman wound up and delivered a high inside fastball to Tony Gwynn, Jr. that was clocked at a blistering 105.1 MPH. The pitch was a ball, but is to date the fastest recorded pitch in MLB history.
